Full description of the position

Full time position Monday - Friday. The Community Liaison reports to and functions under the general direction of the Chief Development & Clinical Care Transitions Officer. The Community Liaison is a mobile position, requiring travel across the entire agency service area. The Community Liaison performs a variety of duties including educating physicians, discharge planners and other potential referral sources.

Responsibilities

includes the field research needed to gain market share/new referral sources, effectively marketing and selling all agency programs, as well as facilitating the admission of patients to CCVNA HealthCare services. As a key member of the health care team, the Community Liaison must interact courteously and effectively with referral resources, as well as with co- workers from all agency departments to facilitate safe and efficient patient care.

• Bachelor's degree in a related field preferred or equivalent experience
• At least two (2) years prior healthcare, marketing or sales experience
• Independent, organized and self-directed
• Good written and verbal communication skills
• Strong customer service skills
• Strong computer skills
• Flexibility with schedule
• Ability to read and understand medical charts.
• Documentation skills for referral tracking purposes.
