Palms Home Care is a growing home health agency dedicated to providing quality in-home care to patients in the communities we serve. We're looking for a motivated Business Development Liaison to build and strengthen relationships with referral sources and help us reach more patients who need our services.

Responsibilities

• Cultivate and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral sources
• Educate referral partners on Palms Home Care's services, admission criteria, and the referral process
• Track territory activity, referral trends, and conversion rates; report on growth opportunities
• Coordinate with intake and clinical teams to ensure smooth, timely admissions
• Represent Palms Home Care at community events, health fairs, and networking functions
• Identify new business opportunities and help develop strategies to expand market share within assigned territory

Qualifications

• 1+ years of business development, marketing, or liaison experience in home health, hospice, or a related healthcare setting (required or strongly preferred)
• Established relationships with local referral sources a plus
• Strong communication and relationship-building skills
• Self-motivated, goal-oriented, and comfortable working independently in the field
• Valid driver's license, reliable transportation, and willingness to travel within assigned territory
• Knowledge of home health regulations, PDGM, and the referral/intake process preferred
