Choice Health at Home is seeking an experienced and motivated Business Development Liaison with an established book of business in home health or hospice to help expand access to high-quality home health services throughout the community. In this role, you'll build relationships with physicians, hospitals, case managers, skilled nursing facilities, and other referral partners while serving as a trusted resource for home health solutions. If you have a passion for healthcare sales, relationship building, and helping patients receive the care they need, we'd love to meet you.

What You'll Do

• Develop and maintain referral relationships with healthcare providers and community partners
• Promote Choice Health at Home as the preferred provider for home health services
• Identify new business opportunities and referral sources
• Collaborate with clinical and operational teams to support patient admissions and care transitions
• Educate referral partners on services, programs, and patient eligibility
• Ensure compliance with all federal, state, and local marketing regulations
• Participate in community outreach, networking events, and professional organizations
• Meet and exceed monthly admission and growth goals
• Provide market feedback and recommendations for service expansion What We're Looking For
• 2+ years of home health or hospice sales experience preferred
• Strong relationship-building and communication skills
• Experience using CRM systems preferred
• HomeCare HomeBase experience preferred
• Valid driver's license and auto insurance required
• Proven ability to meet sales and referral growth goals
